Leadership Development Camp 2017 Camper Application


Thank you for your interest in LDC 2017!

The Red Cross Leadership Development Camp is a 4-day, 3-night leadership conference for 150 youth ages 13 - 17, and entering grades 8 - 12.  This year’s camp will be held on the campus of Xavier University, from July 13 - 16.  Camp fee is $225 with need-based financial assistance available on a limited basis.

Now in its 34th year of impacting young leaders, LDC is designed to give teens experience with and insight into leadership skills, traits and practices.  Our camp is different from other leadership camps in that it is youth-led.  Our peer counselor team has been working throughout the academic year to create the classes, sessions and activities that comprise camp.  They can't wait to be your guides, small group leaders, and class facilitators at camp!

We are looking for campers who are ready to embrace new activities, meet new friends from throughout the tri-state area, and interact with leadership ideas that will stretch them out of their "comfort zone" and into a zone of growth, learning and change.  Campers say they come away from their LDC experience ready to take on new leadership challenges they never dreamed possible – at school, in their community, in their circle of friends and in their volunteer lives.  And – oh yeah! – did we mention that camp is also a TON of fun?!

We want you to know a couple of things as you begin to work  on your application.

The application consists of several parts:

  • Basic Information: Simple questions like your name, address, parent/guardian contact info, etc.  You got this!
  • Getting to Know You: Learning your thoughts on camp and leadership: 2 essay questions, each with a minimum of 150 words.  We recommend that you compose your essays in a Word document and then copy and paste into the application.  Applications with essays containing fewer than 150 words each will not be considered.
  • Information for your parent/guardian to complete:  He or she will need to provide information on your health history and insurance information, and read and approve some legal permission statements.  Parents/guardians will also need to complete the financial assistance form, if assistance is being sought. 
  • Class selection: Tell us what classes you'd like to take at camp.  We can't promise you'll get all of your first choices, but we promise to do our best to get you in classes you ranked highly, as long as space permits.  First come, first served!

Once this on-line application is submitted, you have one more step to the application process.  

NEW APPLICANTS must ask 2 references to submit a NEW CAMPER REFERENCE FORM. 

RETURNING CAMPERS must submit a RECORD of SERVICE FORM FOR RETURNING CAMPERS

FINANCIAL ASSISTANCE REQUEST

Camp fee is $225.  Through the generosity of our camp sponsors, we are able to make some camp sponsorships available on a limited basis.  We ask that you consider your need thoroughly, so that we are able to provide assistance for as many campers as possible.  Aid is awarded on a rolling basis until funds are depleted.

Notification regarding the award of sponsorship funds will be sent along with the Camper Acceptance Packet.

PARENTAL CONSENT

LIABILITY WAIVER STATEMENT

My child, under the age of 18, wishes to participate in the Leadership Development Camp to be held by the American Red Cross at Xavier University from July 13 - 16, 2017.  I am aware that participation in the event is potentially hazardous and entails a risk of physical injury.  I understand and agree that my child is participating at his own risk.  If I am aware of any physical or medical condition that would interfere with my child's ability to participate, I have listed it in the section requesting that information.

IN CONSIDERATION OF MY CHILD BEING PERMITTED TO PARTICIPATE IN THE ACTIVITY, I HEREBY RELEASE AND DISCHARGE THE AMERICAN RED CROSS, THE AMERICAN RED CROSS OF CINCINNATI AND OHIO RIVER VALLEY AND XAVIER UNIVERSITY AND ALL OF THEIR EMPLOYEES, VOLUNTEERS, OFFICERS AND REPRESENTATIVES FROM ANY AND ALL CLAIMS FOR PERSONAL INJURY, DEATH, OR PROPERTY DAMAGE ARISING FROM OR IN ANY WAY RELATED TO MY CHILD'S PARTICIPATION IN THE ACTIVITY, EXCEPT WHERE THE SAME IS CAUSED BY WILLFUL MISCONDUCT OR GROSS NEGLIGENCE.

I hereby consent to his/her participation in the Activity.  If my child is injured or becomes ill and neither I nor the emergency contact listed herein can be reached, I grant permission for agents of the American Red Cross to seek medical attention for my child.  

By checking the box below I affirm that I have read, understand and agree to the above liability waiver.

PHOTO RELEASE

I understand that my camper may be photographed during the course of this Activity. I grant full and unlimited permission to the American Red Cross and its agents of affiliates to use my camper's name, photographs or any other record of participation in this Activity in any broadcast, telecast or other visual or audible record of this event for agency purposes, without compensation.

By entering "YES" below, I grant permission for my camper to be photographed/recorded as stated above.

By entering "NO" below, I deny permission for my camper to be photographed/recorded as stated above.

 

CLASS SELECTION

In addition to other large and small group sessions, campers will participate in one Core Class (2 hours) and two Elective Classes (1 hour).  Every attempt will be made to accommodate camper's class preferences, as space permits.

CORE CLASS SELECTION

You will be taking one of these 2-hour classes at camp.

Once submitting this form, you have one more thing to submit in order for us to consider your candidacy.

IF YOU ARE A NEW CAMPER: Please provide two NEW CAMPER REFERENCE FORMS

Ask two adults who know you well and are NOT related to you to complete the NEW CAMPER REFERENCE FORM. Examples of people to consider as references: a teacher, a neighbor, a coach, someone you've worked for (either a job supervisor or an adult who has employed you to babysit, mow lawns or do other work for them).  You will be able to forward the link to your references and when they complete the form, it will come directly to us.

IF YOU ARE A RETURNING CAMPER: Please provide a RECORD OF VOLUNTEER SERVICE.  

If you received financial assistance to attend camp, please submit a record of TWELVE HOURS of volunteer service. 

If you did NOT receive financial assistance, please submit a record of SIX HOURS of volunteer service. 

We are accepting campers on a rolling basis.  We will not consider your candidacy until we receive your 2 NEW CAMPER REFERENCE FORMS (for new campers)  or your RECORD OF VOLUNTEER SERVICE (for returning campers).
