DCS-PE: Map Your Neighborhood Request Form

Neighborhoods that are prepared for emergencies and disaster situations save lives, reduce the severity of injuries and trauma, and reduce property damage. In addition, contributing as an individual and working together as a team helps develop stronger communities and improve the quality of life in the community. Learn the first 9 Steps to take immediately following a disaster to secure your home and protect your neighborhood.

We have three options within this request form. Please read each option before making your request

  • MYN Overview- A 10-15 minute presentation giving an overview of the program. Choose this option if you have a group of at least 10 people who are interested in learning more about the MYN program.
  • MYN Materials- If you are ready to host a Map Your Neighborhood meeting in your community, select this option. You will receive up to 40 shingles and a DVD (as needed). Only select this if you have attended a 2 hour MYN TTF training class. You will need to provide the date of the facilitator training you attended, the date of your meeting with your neighbors, along with the address range of the area you will be mapping.
  • MYN TTF- Map Your Neighborhood Train the Facilitator. This two hour training will prepare individuals to facilitate the Map Your Neighborhood meeting and implement the program in their neighborhood. Participants will learn the 9 steps to take immediately following a disaster, develop  inventory of neighborhood skills and equipment, map your neighborhood to identify areas of concern such as natural gas meters, and verify neighbors that may need extra help in a disaster- such as the elderly, those with a disability, or children who may be home alone.
